A critical incident analysis and reflection The purpose of this essay is to reflect and critically study an incident from a clinical setting whilst using a model of reflection. This will allow me to analyse and make sense of the incident and draw conclusions relating to personal learning outcomes. The significance of critical analysis and critical incidents will briefly be discussed followed by the process of reflection using the chosen model. The incident will then be described and analysed and the people involved introduced and then I will examine issues raised in light of the recent literature relating to the incident. My essay will include a discussion of communication, interpersonal skills used in the incident, and finally evidence based practice. I will conclude with explaining what I have learned from the experience and how it will change my future actions. The Gibbs model (1988) of reflection cycle will be attached as appendix 1 and description of incident will be attached as appendix 2. In accordance with the 2004 Nursing and Midwifery Council, the clients details and placement setting has not been disclosed in order to maintain confidentiality. Critical incidents are snapshots of something that happens to a patient, their family or nurse. It may be something positive, or it could be a situation where someone has suffered in some way (Rich Parker 2001). According to Hogston and Simpson (2002) reflection is a process of reviewing an experience of practice in order to better describe, analyse and evaluate, and so inform learning about practice. Wolverson (2000) includes this is an important process for all nurses wishing to improve their practice. This will be investigated using a reflective nursing model. I am going to use Gibbs (1988) Reflective Cycle. This because Gibbs is clear and precise allowing for description, analysis and evaluation of the experience helping me to make sense of experiences and examines my practice. However Ghaye and Lillyman (2006) state that it is miscontructed as ideal for only negative experiences. On the other hand they emphasise that it its strengths lies with the incorporation of knowledge, feelings and action in one learning cycle. Taking action is the key; Gibbs prompts to formulate an action plan. This enables to look at my practice and see what I would change in the future, how I would develop and improve my own practice. Gibbs (1988) consists of six stages to complete one cycle which is able to improve my nursing practice continuously and learning from the experience for better practice in the future. The cycle starts with a description of the situation, next is to analysis of the feelings, third is an evaluation of the experience, fourth stage is an analysis to make sense of the experience, fifth stage is a conclusion of what else could I have done and final stage is an action plan to prepare if the situation arose again (NHS, 2006). Baird and winter (2005) give some reasons why reflection is require in the reflective practice. They state that a reflect is to generate the practice knowledge, assist an ability to adapt new situations, develop self-esteem and satisfaction as well as to value, develop and professionalizing practice. However, Siviter (2004) explain that reflection is about gaining self-confidence, identify when to improve, learning from own mistakes and behaviour, looking at other peopl e perspectives, being self-aware and improving the future by learning the past. In my context with the patient, it is important for me to improve the therapeutic relationship which is the nurse-patient relationship. In the therapeutic relationship, there is the therapeutic rapport establish from a sense of trust and a mutual understanding exists between a nurse and a patient that build in a special link of the relationship (Harkreader and Hogan, 2004). Asserive Description This is attached as appendix one. Feelings In this paragraph, I would discuss on my feelings or thinking that took place in the event happened. I was shocked that the doctor did not wash her hands or use alcohol prior examining Ms Adams especially with all the infection control guidelines and protocols in place. In spite of this I did not have confidence and felt intimidated due to the fact the doctor was more knowledgeable and experienced than I was as a first year student, also I did not want to make him feel uncomfortable. Furthermore I did not want the patient to feel alarmed and worried by challenging the doctor whilst Ms Adams was there. However soon after I had a word with my mentor and told her what I observed and she then recommended that together we confront the doctor, therefore the next day my mentor spoke to her in private and she asked her, if before examining Ms Adams whether she washed her hands. The doctor seemed stunned by this conversation but admitted she did not wash her hands. She responded by justifying his actions and saying he was busy and was in a rush to remember. My mentor discussed the significance of infection control and hand hygiene and then the doctor promised her that she would make sure she follows the protocols and cleanses her hands prior examining any patient in the future. Evaluation This event was difficult and challenging for me as I felt disappointment for my lack of confidence in not confronting and challenging the doctor prior him examining Ms Adams, on the other hand I felt content in the way the doctor responded so positive and optimistic. Consequently I observed that doctor has now changed his practice as a result of this incident. I have learnt from this incident the importance of acting assertively with staff members in a sensitive approach in order to safeguard patientà ¢Ãƒ ¢Ã¢â‚¬Å¡Ã‚ ¬Ãƒ ¢Ã¢â‚¬Å¾Ã‚ ¢s health. Analysis Nurses have a responsibility to safeguard and promote the interests of individual patients and Clients (NMC 2004). Hand hygiene is a crucial factor in the control of hospital-acquired infection (HAI) because hands can easily transfer micro-organisms from one area or patient to another. According to Shuttlewood (cited in Beckford-Ball, Hainsworth) states that despite strategies promoting hand hygiene there still seems to be difficulty persuading staff to adopt good practice. Doctors are the worst offenders. According to NHS figures, 25% of them fail to follow basic hand-washing procedures, compared with 10% of nurses and 15% of ancillary staff. This reinforces the need to ensure all clothing worn by staff in all clinical areas is fit for purpose and able to withstand laundering. Advocacy ranges from activities on behalf of patients, such as hand washing and proper identification before treatments, to arguing that an early discharge will harm her patients recovery. Following conversation with my mentor acknowledged that I need to develop the confidence to challenge the practice of colleagues, understanding pressures that may be under but ensuring that their practice does not put patients at risk. If a nurse observes a practice or procedure she believes to be wrong, advocating for her patient demands she speak out even if that practice was carried out by her superior. This is not always easy and may have a cost for the nurse. I realise that I need to be supportive to colleagues, understanding the pressures that they may be under, but ensuring that their practice does not put clients at risk. Action Plan My action plan is always to work as part of a team, learn more about how best to communicate in order to contribute to good nursing care. I will aim improve and develop my assertive skills when working with staff members to ensure health and safety of patients is maintained. Therefore I will make this a goal for learning in my next placement and discuss with my mentor to work out strategies for how I can achieve this.

Multicultural Social Studies More now than ever before in the educational history of our nation has multicultural education become important. Due to the ever increasing diversity in our nation, the global economy, and the mobility of our work force, diversity in the classroom is affecting our classrooms. With such a diverse student population teachers must understand that each student will bring his own background knowledge and experience into the learning environment. The task that is presented to teachers is to prepare the students to understand and interact with all the different cultures that are present or that the student may come into contact with, while at the same time teach them content which does not offend a student’s cultural identity. Why is multicultural education important? â€Å"Curriculum Standards for Social Studies (NCSS,1994b) endorses the belief that students should be helped to construct a pluralist perspective based on diversity. Former President Gloria Macapagal Arroyo’s achievements about the economy, I am thankful for Gloria Macapagal Arroyo’s achievements for creating a flawless and awesome job for building and creating for giving employment to millions of people who were jobless In the Philippines, and also thanking her for creating a lot of public schools for a lot of people who cannot afford education, students or young people who cannot afford to go to school because of financial problem.I thought that our former President Gloria Macapagal Arroyo is not great because our former President Gloria Macapagal Arroyo cheated in the election against Fernando Poe Junior. She also cheated in the election by seeking the help from Garcy. Our former President Gloria Macapagal Arroyo is getting money from the government that we were all even trying to pay our taxes for a good econo my even though that our parents were all having a hard time earning such money to pay the tax, but she just stole it from us like it was hers, and that made me realize how pathetic she was.But then her achievements all together are fantastic, like building bridges for a better transportation, building schools for educational purposes, for building the Light Railway Transmit also known as LRT and for helping the flood victims in the Ondoy incident, and also giving respect to our navy, marine soldiers that were killed in Mindanao because of the terrorists. According to her she had made the economy grow about 3. % which is true, in fact we can all feel this growth, by seeing the technology we have now, like high- tech gadgets and a more growing technology, I can say that we have indeed grew economically because of our former President Gloria Macapagal Arroyo works. Maybe if she did not corrupt or stole money, no one would hate her and with her achievements she will be loved by the Fili pino people all across the country.If she did not corrupt she would make a fine president for the Philippines, or stole money or cheat because we could see from all of her achievements that she is a potential president. And I myself as a student is thanking her from my heart because of our former President Gloria Macapagal Arroyo always declared that the classes are suspended!!! That made me has an easy school life and a happy school life that I could enjoy with fully with my friends.Our current elected president of the republic of the Philippines which is President Benigno â€Å"Noynoy† Cojuanco Aquino Junior is indeed not a corrupted president. As time passes by we can see that there has not made any corrupt movements from our President Benigno â€Å"Noynoy† Cojuanco Aquino Junior, he fulfills his promises that he made in the election like, not being a corrupt president, not cheating and a lot more.He even made a lot of improvement in our country like, making the cri me rates go down, making the economic growth improve even more but as what our former President Gloria Macapagal Arroyo said that she even had a better percentage of growth than our President Benigno â€Å"Noynoy† Cojuanco Aquino Junior had made, the reason why the Filipinos elected him because he had a potential in making the Philippines into a better country by being a better president than the past presidents that took the throne in the Philippines.President Benigno â€Å"Noynoy† Cojuanco Aquino Junior, has a little bit of a minor problem even if he does not corrupt from us he himself does not move to fix some problems throughout our country like what former President Gloria Macapagal Arroyo has said that he loves cars, and guns too much that he is not paying attention to the government at all.And also from the videos that I watched that he is not giving the land to the farmers in hacienda Luicita, that was already distributed by former President Gloria Macapagal Ar royo but now our current President Benigno â€Å"Noynoy† Cojuanco Aquino Junior is trying to collect the land or getting the lands that were distributed by former President Gloria Macapagal Arroyo.I don’t know which president is better than the other because the two of them are both amazing in doing their part but is also amazing in corrupting or not moving or doing anything for the economy, I am a student so I may not know all of the things happening in the Republic but a lot of people say that our former President Gloria Macapagal Arroyo is so corrupt that she should be in prison or jailed for life.That is why the Department of Justice is preventing her from leaving the country, named Philippines because she might escape and not come back and it could shame the Philippines. Our freedom to make ethical choices is only an apparent freedom Essay

Do you even know? Is it freedom? Or truth? Perhaps peace? Yes? No? Could it be for love? Illusions, Mr. Anderson. Vagaries of perception. And all of them as artificial as the Matrix itself, although only a human mind could invent something as insipid as love. You must be able to see it, Mr. Anderson. You must know it by now. You can’t win. It’s pointless to keep fighting. Why, Mr. Anderson? Why? Why do you persist?! Neo: Because I choose to. In every person the most basic desire of all is to be free, being able to act at will, not under compulsion or restraint. As I democratic society, we like to think of ourselves of being 100% free, but we are not. In every choice we make there are compulsions and restraints inflicting on the choices we have ever made from the beginning. The older and more independent we appear to be the more of these constraints we are compelled with. These choices we make can be applied to morality. There are many ways in which our freedom, in reality, is limited. By law we are restricted to acts that systems of government deem acceptable. Social acceptance means the majority to act within a bracket of normality and within bounds of what is socially acceptable. If I wanted to go to school naked on every Friday I would socially and lawfully be unable to this. Therefore I am not free to express myself due to external laws forced upon me against my free will. However even though these laws are in place I still have the power to break them therefore making me free. Merovingian: Please Mon Cherie, I have told you, we are all victims of causality. I drank too much wine, I must take a piss. Cause, and effect. Au revoir. Causality also means that I a person can never truly become free. If I already know the consequences of an action then my choice has already confined. For example I would like to throw microwaves off motorway bridges but I know that this may cause innocent motorists to die therefore I don’t. The mind with which we make our decisions has been influenced by a whole range of external factors. These must reduce our moral responsibility and hence be restricting of true freedom. Personal and psychological differences within people will provide them with different abilities and hence different freedoms and limitations. People can only act in the way that their personalities allow them, by causing them to react to situations in a specific way. But what if free will is non-existent and we are only convinced we have self-control because devices in fact control us beyond our own perception. In this case everything is pre determined and the choices we face are pre decided for us leaving us with the illusion that we have chosen ourselves and decided our own fate; exercised free will. Choice is an illusion created between those with power, and those without Hard determinism, considers everything in the present to be directly caused by events that preceded them. Everything including the actions we take and the choices we make are caused directly by another event. Each human mind is the product of its experiences and in every situation will react according to what it has learnt, in a similar way to a computer that has been programmed. However this links back with responsibility, if a human was already pre destined to perform a certain task they should receive no praise for correctness in taking the action, as what they have done was done not out of free will, but because they were programmed, or already decided. The action says nothing about the moral worth of the person as it had an external cause, and was not done through free will and intention. The hard determinist view that everything is decided by a constant line of causes, and that humans are not free simply because every thing we supposedly decide is already caused and so determined, ultimately means that human free will is an illusion. Free will is something we feel we experience when making decisions and choosing but is really non-existent, the actions we partake in are already set and what we feel we decide is irrelevant to anything that actually happens. For example if I sat down in a room I would be free to step out of until I realised the door was locked. I am free to make this choice but when but my choice is irrelevant as I unable to do anything about it. Determinism removes this moral responsibility and so removes ethical decisions. However with freedom comes responsibility. Libertarianism presents the opposite idea that we are completely free therefore giving people total moral responsibility. The existence of human free will are largely based on the defined different between ones personality and ones moral self. While we have a sense of freedom, a sense deliberating over our options. Because of this, they would argue that universal causation is not necessarily relevant to human actions. They do not deny any influence to the human mind that could have an effect on the way in which one might act, but they claim that there is still a large aspect of freedom of choice involved. As with all workable theories finding the right balance is essential. Soft determinism is the third deterministic view and one where determinism and free will are completely compatible. It describes that we are morally responsible for our action although some are determined. Therefore the decisions we are free and able to make in our own minds count as the causes by which everything is made to occur. The midway position suggests that some of our actions are conditioned while others have a complex number of causes. For example there could be a number of reason why someone does not eat food, whether it be a diet, religious beliefs, famine or lack of money. â€Å"Real freedom,† in the question seems to suggest the freedom to take these fully conscious, and reasoned ethical decisions without relying a higher power. To conclude I believe that as human beings we are not free. Our behaviour and morals will always be determined social acceptance, laws, causality, and upbringing. However we have the power in ourselves to break down these constraints and become free, if a door is locked then break it down, if I want to throw microwaves of motorways then I will do so. When we become totally free this leads to anarchy and chaos. When it comes down to it is not a case of whether we are free or not it is whether we choose to be. Architect: Precisely. The Crazy Eddies financial statements had many fraudulent over and understatements done in many ways that the auditors should have caught. They created fictitious revenues by a number of means. They prepared phony invoices showing sales which overstated their revenues to show the company was growing faster than they actually were. Their vendors collaborated in the fraud by lying to the auditors when the auditors attempted to confirm some of these receivables. The auditors were not diligent when they verified these invoices. They should have probed further into the vendors to verify that these sales occurred. They also should have understood the relationship between Cray Eddies and their vendors to understand if there were motives for fraud. They overstated their assets by overvaluing inventory. They would borrow merchandise from suppliers to inflate the ending inventory. The suppliers would ship the merchandise to the Crazy Eddies stores and hold the billing until after the end of the accounting period. The employees of Crazy Eddie went to great extents to deceive the auditors. They would move inventory to the stores or warehouses that were being audited to conceal the shortages. The auditors should have caught that the merchandise was not billed and understood what accounts they charged in the books when the merchandise was received. Another means of overstating the inventory was they shipped inventory from one store to another store so it could be double counted. This should have been caught by the auditors by having the entire inventory verified in parallel. The employees included in their inventory consigned merchandise and goods being returned to suppliers. This could have been found by understanding the details of Crazy Eddies inventory. The auditor should have identified the consigned merchandise and goods being return to separate it from the normal inventory. Crazy Eddie used the accounting periods to overstate assets and income. They held off closing the books past the end of the accounting period to overstated assets and income by boosting sales. The other means used was to reduce liabilities and expenses by not recording them until the next period. The auditors should have verified books at the end of the accounting periods to make sure that all transactions were recorded. The auditors needed to verify the transactions around the end of the period to verify their timing accuracy. Another category of fraudulent activities was when they were completing their financial statements. They didnt adequately disclose facts in the financial statements according to GAAP. The footnote during one period stated that certain income was recognized when received and the following period disclosed that income was recognized when earned. The auditors should have added an explanatory paragraph or a modification of wording for lack of consistent application of GAAP. Main Hurdman was the auditor for the companys annual audits and its consulting division was charging Crazy Eddie millions of dollars to computerize Crazy Eddies inventory system. This violates rule 102 for integrity and objectivity. Auditors cant objectively audit an inventory system they developed which means this is a conflict of interest. They should have chosen to develop the inventory system or be the auditor but not both. Crazy Eddies employees later stopped using the computer based inventory system designed by Main Hurdman and used their manual inventory system. The auditors should have noticed the change in the inventory system being used and understood why they changed. This should have been a signal that they were removing some of the quality controls in the inventory system. Many of Crazy Eddies accountants were former members of Main Hurdman accounting firm. The SEC added a requirement in the Sarbanes-Oxley Act that requires a member of the audit team not work at the client for one-year in certain key management positions. The firm is not independent with respect to the audit client if a former auditor accepts employment with the client. There were several reported instances where the auditor requested the client documents and were told that those documents were lost or inadvertently destroyed. Sarbanes-Oxley Act states that all working papers must be retained for 7 years. The inadvertent destruction of working papers that are less than 5 years has a penalty of 10 years in prison. The collusion of key personnel at Cray Eddies made it very difficult for auditors to uncover the fraud. If the auditors would have done a risk assessment prior to engaging with the client they would have identified a risk with so many family members employed at the company. The success and growth rate of Crazy Eddie was not normal. The auditors should have completed a comparative analysis of other electronic retailers. This would have shown that their high growth rate wasnt typical compared to others. This would have prompted the auditor to look closer at why they were outperforming the competitors. To prevent the CPA firm from having this occur in the future, they should follow the elements of quality control. The one element is the policies and procedures before accepting or continuing a new client relationship. They should complete a client evaluation form which includes evaluation of the clients management. If this was done in the Crazy Eddies case, they would have found that it was a risky client based on his selling techniques alone. The firm should follow the element of personnel management where all new personnel should be qualified to perform their work competently. The personnel should have adequate training and proficient. The field auditors for Crazy Eddies were young and inexperienced. The management team was able to manipulate and influence the auditors. The auditor needs to follow the procedure of verifying the inventory count by observing it. Precautions must be taken to avoid allowing the inventory to be moved by verifying it in parallel if multiple locations exist. Crazy Eddies auditors were fooled by not taking precautions to avoid double counting the inventory. The firm must follow the Generally Accepted Auditing Standards. One of the standards of field work is to obtain a sufficient understanding of the entity and its environment including its internal control. If this was done, the auditors could have seen that Crazy Eddie had influence over his suppliers to cooperate in inflating the assets. The change in the inventory system back to the manual system showed a lack of internal control. The firm must follow the rules of conduct. They must keep their objectivity by not providing other services such developing or advising an inventory system. They must remain independent by not auditing a client that has former members of the audit team working at the client within one-year.

In 1956 his aunt bought John a guitar that started he shows his music ability and then he created the group called the ?gQuarry Man?h. The band had shifting member until 1957 when the second permanent member was in. His name was Paul Mccartney. John came up with the name Beatles for the group. They were performing in the cavern. In 1961 the Beatles debuted at the Cavern Club and then they released their first single ?gLove me Do?h. John met a woman in 1966. Her name is Yoko Ono. John fell love with her and he divorced his first wife and re-married Yoko Ono. After the Beatles broke up and John started doing this by releasing solo album Imagine. But on December 8, 1980,Lennon, returning to their apartment on New York City?

  Ã‚  Ã‚  Ã‚  Ã‚  I slam my alarm clock to shut it off. I try to open my eyes to see what time it is, but it feels as though the sleep fairy came along while I was sleeping and super glues my eyes shut. I knew I had to get up to go to the gym, so I rolled over, stood up and flicked the light on. At that moment it felt like I was staring directly into the sun, every time I tried to open my eyes they would close immediately. I am still not awake yet but I know if I want to go to the gym before work I have to push myself to wake up. Stumbling all over my room, I try to throw on my gym clothes in record-breaking time. Finally I am ready to go. I try to remember not to forget anything as I mumble to myself, â€Å"I grabbed my clothes for work, my gym bag, my wallet, my keys, and my cell phone.† I slam the door and fly out the house getting everything in about ten minutes.   Ã‚  Ã‚  Ã‚  Ã‚  Outside it is so cold, my body starts to shiver on the way to the car sitting about twenty feet from my front door. Getting into my car is the worst part of my morning routine. I dig my hand deep into my pocket to pull my keys out that seem to be all tangled up in the inner lining, which makes my time out in the freezing cold longer. Hopping into my car and shutting the door figuring I will be much more comfortable, which wasn’t true because the inside of my car also felt like a freezer. All I can think about at the moment is turning on the car and wishing it would warm up faster than it had the day before. Everyday during the winter I have to wait longer for the car to warm up for the fact that there is so much ice on my windshield, I know I cant just start driving because I cant see a thing.

Managing Outbreaks of an Infection Essay

NICE and Department of Health Guidelines NICEandDepartment of Health guidelines set the standards for high quality healthcare. It encourages a healthy lifestyle for people. The NICE and Department of Health guidelines are also used by the NHS, Local Authorities, employers, voluntary groups and anyone else involved in delivering care and promoting wellbeing. The recommendations that are given provide guidance on infection control and taking precautions. These should be applied by all healthcare personnel, and other carers, to the care of patients in all healthcare settings. For example, hands must be washed immediately before each and every episode of direct patient contact or care, and after any activity or contact that could potentially result in hands becoming contaminated. Another example is, when handling and disposing sharp objects. Sharps must not be passed directly from hand to hand and handling should be kept to a minimum. Also needles must not be recapped, bent, broken or opened before use or disposal. This is to ensure that infections do not spread and no one gets hurt with the needles. When it comes to precautions then these guidelines say that precautions should be taken with care. Health care professionals should ensure that these precautions benefit the patients who have TB and also prevent anyone else from getting these ill. For example, isolation nursing. The guideline says that this should be done to prevent anyone else from catching the infection and to stop the infection from spreading. What is Tuberculosis (TB)? Tuberculosis (TB) is a bacterial infection that is spread through inhaling tiny droplets from the coughs or sneezes of an infected person.It is a serious condition but can be cured with proper treatment. TB mainly affects the lungs but it can also affect any part of the body, including the bones and nervous system. In some cases the bacteria infect the body but do not cause any symptoms which are known as latent TB. It is called active TB if the bacteria causes symptoms.  A TB infection of the lungs is known as pulmonary TB but TB can occur outside the lungs, which is known as Extrapulmonary TB. What are my Concerns? My concerns are that the TB infection can spread from four patients to more. TB is caused by a type of bacterium called mycobacterium tuberculosis. TB is spread when a person with the TB infection in their lungs coughs or sneezes. Someone else needs to inhale and take in the droplets containing the TB bacteria. To catch it from someone else you would usually have to spend a long period of time in close contact with the infected person for you to catch the infection. For example, TB is more likely to spread between family members who live in the same house. This is because family members are living and are around the infection person mostly. This means they are at more risk of catching the infection. It would be very unlikely for someone to become infected by sitting next to an infected person on a bus. It is my concern that some of my other patients will be able to catch the infection from the four patients who already have TB. This is a concern because we do not want all the patients within our ward being infected with TB. TB is a serious infection. Why I need to prevent an outbreak? I need to prevent this outbreak because TB is an infection which could harm the person who has it. If untreated the patients can become worse. Also we need to prevent it from spreading. This is done to ensure that other people do not catch the infection. Also when one person has the infection then it will keep spreading and everyone in the hospital will be sick. This will be done by isolation. This is when we isolate the patient away from the other patients. This ensures that this infection is not able to spread to anyone else. This means the infection will keep spreading. The bacteria enters our body and once it has entered our body it starts to multiply and because it is a bacterial infection it does not need to attach to another cell to multiply. It is able to multiply on its own. After multiplying it starts to cause mycobacterium infection. Our immune system cannot stop the TB bacteria from growing and spreading after the infection has started to spread into our body. Medication and treatment is then needed for the body to then recover. Preventing an outbreak is important because it can spread and start  to infect other people. If a person is around someone that has been infected then they need to make sure that you stay away from them. This is because the individual is able to catch the infection from affected items and affected people. The NICE guidelines:â€Å"It has long been recognised that people who are sputum microscopy positive from spontaneously expectorated sputum are those cases with the highest infectivity, and pose a risk to household and other close contacts such as workplace contacts. For these reasons, traditionally, patients with pulmonary disease in whom tuberculosis is suspected are isolated in a single room.† Isolation nursing is one way in which an outbreak of infection can be prevented. The Department of Health guidelines:â€Å"This section discusses the evidence and associated recommendations for the use of person al protective equipment by healthcare workers in general care settings, including aprons, gowns, gloves, eye protection and face masks. Where appropriate, in addition to the grade of the evidence underpinning the recommendations, there is an indication of a Health and Safety requirement. The decision to use or wear personal protective equipment must be based upon an assessment of the level of risk associated with a specific patient care activity or intervention and take account of current health and safety legislation.† Using personal and professional equipment is another way in which an outbreak can be prevented. The doctors, nurses and other professionals working in a hospital should make sure that they do everything that prevents the infection from spreading to the workers, all the patients and visitors. Preventing an outbreak is important because we do not want other people getting ill. TB is a serious bacterial infection. Preventing other people from getting it is important. What are the risks and damage of an outbreak on staff, patients, visitors and their families? The risks of this are that staff, patients, visitors and their families are able to catch the TB illness. This is a risk because we will have more patients in the hospital with TB. Once a person is infected with TB bacteria, the chance of developing TB disease is higher if the person; has HIV infection, has been recently infected with TB bacteria (in the last 2 years), has other health problems, like diabetes, that make it hard for the body to fight bacteria, abuses alcohol or uses illegal drugs; or was not treated correctly for TB infection in the past. The damage of  this is that it then damages the individual’s body. This is because they go into getting further illnesses in the future. The department of Health Guidelines: â€Å"The incidence of TB is influenced by risk factors such as exposure to, and susceptibility to, TB and levels of deprivation (poverty, housing, nutrition a nd access to healthcare), and differs in different parts of England and Wales. Where scientific evidence supports it, this guideline makes recommendations on service organisation, as well as for individual teams of healthcare professionals. The guideline aims to focus NHS resources where they will combat the spread of TB, and some sections deal with high- and low-incidence areas separately. The NICE guidelines: â€Å"Anyone exposed to TB bacteria can become infected but people at particular risk are those that are less able to fight infection. Those at risk include: Household and frequent contacts of infectious cases, those who have lived in, travel to or receive visitors from places where TB is still very common, those who live in ethnic minority communities originating from places where TB is very common, those with immune systems weakened by HIV infection, the very young and the elderly, as their immune systems are less robust, those with chronic poor health and nutrition because of lifestyle problems such as homelessness, drug abuse or alcoholism, those living in poor or crowded housing conditions, including those living in hostels. Other conditions that suppress immunity such as renal failure and chemotherapy and diabetes increase the likelihood of reactivation of TB.† What precautions do we need to take? For example, if we wear a duck mask then it blocks our nose and mouth. These are two portals in which infections can come into a person. That I why blocking these two portals of entry is important so the infection is not able to get into people. If a nurse or doctor does go into the room to then they need to make sure they use personal and professional equipment. This will ensure that they are protected from the infection. The NICE guidelines: â€Å"Healthcare workers caring for people with TB should not use masks, gowns or barrier nursing techniques unless: MDR TB (multidrug-resistant Tuberculosis) is suspected or aerosol-generating procedures are being performed. Also the patients clothes and linen should be washed separately from the other patients linen and clothing. This will ensure that infections do not spread. Each level operates at a different point in the transmission process: administrative control measures reduce HCW and patient exposure; environmental control measures reduce the concentration of infectious droplet nuclei, personal protective devices (respiratory protection) protect HCWs in areas where the concentration of droplet nuclei cannot be adequately reduced by Administrative and environmental control measures.† How would we educate staff, visitors and patients? Education is vital for people. If we educate staff, visitors and patients about TB then they would be more aware of what TB is, how it affects the body and how to prevent yourself from getting TB. We could do this through putting up posters around the ward and hospital. We could also make leaflets and hand them out to patients and visitors around the hospital. Also doctors and  nurses could speak to family members and friends and tell them about TB. Experience suggests that targeted campaigns tailored to the local population work better than national campaigns. Keeping GPs aware of prompt referral systems. Resourcing TB services adequately so that they are not only involved in teaching clinical colleagues, but also in raising awareness of TB, particularly among high-risk groups; and informing clinicians of peer group networks and encouraging them to seek advice on treatment where appropriate.
